Rock Chip Repair

By purchasing this product, I affirm that I have authorized Today AutoGlass (Lumin AutoGlass LLC) to repair the windshield in the vehicle listed above. If I have chosen to use my insurance to cover the repairs listed above, I authorize my insurance provider to make payment to Today AutoGlass. If I receive payment from my insurer, I agree to forward that payment to Today AutoGlass (Lumin AutoGlass LLC) equal to the amount I receive. I understand that the rock chip repair process is primarily performed to stop the chip from spreading and the repair may remain partially visible. Today AutoGlass accepts no responsibility for runs or cracks during the repair process. Should the chip spread after completion, Today AutoGlass will repair, or apply repair cost credit toward the replacement of the windshield. 

I agree to defend, indemnify, and hold harmless Today AutoGlass (Lumin AutoGlass LLC) from any and all claims, lawsuits, demands, causes of action, liability, loss, damage and/or injury, of any kind whatsoever (including without limitations all claims for monetary loss, property damage, equitable relief personal injury and/or wrongful death), whether brought be an individual or other entity, or imposed by a court of law or by other administrative action of any federal, state, or local governmental body or agency, arising out of, in any way whatsoever, any acts, omissions, negligence, or willful misconduct on the part of Today AutoGlass (Lumin AutoGlass LLC), its offices, owners, personnel, employees, agents, contractors, invitees, or volunteers. This indemnification applies to and includes, without limitation, the payment of all penalties, fines, judgments, awards, decrees, attorney’ fees, and related costs or expenses, and any reimbursements to Today AutoGlass (Lumin AutoGlass LLC) for all legal fees, expenses and costs incurred by it.
